Reactive lymphocytes are ordinary lymphocytes that have changed their appearance because the immune system has been activated, most often by a viral infection. When a lab flags them on a manual blood film, it usually means your body is mounting a normal response to a recent trigger rather than signaling anything sinister. Context, and the rest of your blood count, determine what the finding actually means for you.

What reactive lymphocytes measure
Reactive lymphocytes, sometimes called atypical lymphocytes, are activated versions of the lymphocytes already circulating in your blood. When these cells encounter an antigen, they enlarge, their cytoplasm expands and takes on a bluer tone, and their nucleus loosens in structure. A laboratory scientist identifies them by eye during a manual differential on a stained blood smear. They are not a separate cell line; they are your existing T and B lymphocytes caught in the act of responding to a challenge.
What it means when reactive lymphocytes appear
The most common reason reactive lymphocytes show up is a viral infection. Classic examples include infectious mononucleosis caused by Epstein-Barr virus (EBV), as well as cytomegalovirus (CMV) and other common viruses. Recent vaccination or certain medication reactions can produce them too. In these settings the cells reflect a healthy, appropriate immune response: your lymphocytes are working, and the finding is usually benign and self-limited, resolving as the infection clears. What matters is the broader picture. A modest rise alongside symptoms of a viral illness is reassuring, whereas cells that are persistent, very numerous, or accompanied by other abnormal counts prompt a clinician to look further and interpret the result in full context.
